Office Hours

Our office hours are 8am to 5pm Monday through Friday. Patients are seen by appointment. New patients should arrive a half hour before scheduled appointments and have related x-rays, medical records and insurance cards. We make a sincere effort to adhere to appointment schedule, however if there is a delay due to emergency or surgery, we will offer the option to reschedule appointment. Please notify office if appointment cannot be kept.

OSMSCI offers complimentary parking in the front and rear of our building (Kirkwood Bend). In the event that you park in the rear, you will enter through our buildings common area and veer to the right when you come to a large staircase, which will lead you directly to our office.

OSMSCI has two wheelchairs to assist you from your car to our office or vice versa. They are located in the front of our building in the vestibule. Please notify our receptionist in the event they that are both in use.



Requests for all prescriptions and refills must be made during normal office hours. When you need medication refills please contact your pharmacy at least two days in advance. Narcotic refills cannot be filled outside office hours. Please be advised that some medications require a written prescription. If this is the case, it will be mailed to you or may be picked up at our office.


Insurance Information

We participate in most insurance and managed care plans. As providers for these carriers, we are happy to file insurance claims for patients. Co-payments, deductibles and payment for any non-covered services are due at appointment. If insurance requires a referral or authorization prior to treatment, patient will be responsible for obtaining information from primary care physician. If insurance carrier requests additional information, please provide information as soon as possible so that the claim can be paid in a timely manner. In the event treatment is a result of an auto accident or other third party liability, we will be glad to file a claim to patient’s group health insurance carrier, but we do not accept third party liability insurance. Our billing staff will be glad to assist in answering questions regarding coverage, billing and insurance or if special arrangements need to be made. We accept Visa, MasterCard, American Express and Discover.


Patient Education / Presentations

We believe that providing educational and informative materials to our patients and their families allows and assists in making decisions about proposed testing, surgical and other treatment options in an informed and confident manner. When you come for your appointment please remember to obtain and bring the following:

  • Drivers License or a valid ID
  • Insurance information
  • Referral Letter (if required)
  • Reports, X-rays, MRI’s, CT scans etc and any other relevant information
  • List of medications (if any)

If you need to cancel an appointment

Telephone the office during business hours and allow at least 1 day’s notice so that we can offer your appointment time to patients on our waiting list.

We recognize that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.
